Change And Renewal of Zambian Passport


All Zambian Passport holders are notified that the exercise of change of passports from the old type to the new type of passports with enhanced security features which commenced in September 2008 is on going.

The Zambian High Commission is pleased to inform all Zambians resident in Japan, Papua New Guinea, Solomon Islands and New Zealand that the Mission is currently receiving applications for onward transmission to Lusaka.

  •  Passport Fees

    The costs for change of Passports are as follows:

  •  Payment by Direct Deposit:

  •  Passport Application Form

    Application for applicants above 18 years of age -  Form 'A'  |   Form 'N'

    Application for applicants below 18 years of age -  Form 'B'   |   Form 'M'

  • However, note that there is one (1) type of form that can not be downloaded. It is only sent to the applicant after receipt of money. It is a bar coded form with a serial number. The High Commission posts this to respective applicants in its original form and should be returned in the same manner.


  •  Mode of Application

    Adult Applicants - above 10 years of age

    • A duly completed form 'A'
    • A duly completed form 'N'
    • Three (3) recent Passport sized photographs, one of which must be endorsed by the Recommender
    • A photocopy of Birth Certificate or Affidavit of Birth
    • A photocopy of National Registration Card (both sides)
    • Photocopies of Passport (Data page, pages 1-4 and last 2 pages)

    APPLICANTS - under 18 years of age

    • A duly completed form 'B'
    • A duly completed form 'M'
    • Three (3) recent Passport sized photographs, one of which must be endorsed by the Recommender
    • A photocopy of Birth Certificate
    • A letter of consent from either Parent
    • A photocopy of either parent's Passport
    • A photocopy of either parent's National Registration Card (NRC)

  •  Travel Document Of Identity:
    Travel document of identity will be issued in cases of emergency travel only and will be valid for six (6) months only and non-renewable. It is basically a one way travel document to Lusaka

  •  Replacement Of Passport
    • A duly completed form 'A' and 'N' for adults and 'B' and 'M' for those below 10 years
    • Three (3) recent Passport sized photographs, one of which must be endorsed by the recommender
    • A duly completed Lost Passport Description Form 'L'
    • Police report on the lost Passport
    • Lost Passport Affidavit
    • A photocopy of National Registration Card, Birth Certificate or Affidavit of Birth
    • A photocopy of Passport (if any)

  •  Changes Of Names
    • A photocopy of any of the following: Marriage certificate, Divorce certificate or Deed poll
    • A duly completed Form 'E'

  •  Mode Of Submission
    • Request for application forms should be accompanied by a pre-paid self-addressed envelope.
    • Validity of passport application is Adults above 18 (in several places) years and Children below 18 years.
    • All photocopies must be authenticated as true copies of the original documents by the commissioner of Oaths.
    • The validity of new Passports for adults will be for ten (10) years and five (5) years for those below eighteen (18) years of age.

  •  Processing Time

    A minimum of ninety(90) days will be required from receipt of the application.

    N.B. Zambian passports are issued in Zambia at the following centers: Lusaka, Ndola, and Livingstone.


  •  Requirements For Application Of Zambian Passport
    • Form A and N for applicants above 10 years of age
    • Form B and M for applicants below 10 years of age
    • Copies of applicant's or parent's/guardian's National Registration Card (NRC)
    • Copies of pages:
      • 4.1 Copies of pages 1,2,3 of old Passport
      • 4.2 Copies of pages 1,2,3 of the passport of the guardian
      • 4.3 Copies of pages 1,2,3 of passport or guardian's Travel Document of Identity Form 'G'
    • Three (3) full recent photos without hat, showing full face, with white background and should not be mounted, one of which must be endorsed by the Recommender.
    • Copy of Marriage Certificate for married applicants.
    • For persons born in Zambia of foreign parents, they should produce proof of residential status of their status of their parents/guardians in Zambia at the time of birth of the applicants.
    • Any person claiming legal custody of the children under twenty one (21) years other than biological parents should produce court orders or relevant legal documents committing the child to that person's custody.
    • Persons born of foreign parent should produce documentary proof to show that one of the parent is a Zambian citizen. (copies of N.R.C and Passport)
    • Travel documents of identity should be issued in cases of emergency travel only, valid not exceeding six (6) months of the one-way validity clause to persons returning to Zambia.
    • All photocopies must be authenticated as true copies of the original relevant documents, by a Commissioner of Oaths.
    • A change of the name other than by marriage or adoption must be substantiated by the production of evidence showing that a bona fide change has been made for all purposes.
